LINKED BY LOVE 2024 VACATION BIBLE SCHOOL REGISTRATION AND CONSENT FORMS
Join us for Linked By Love Vacation Bible School, June 17 – 21, including a Juneteenth celebration.
This outdoor event is hosted by United Presbyterian Church and staffed by camp leaders from the Clearwater Forest Camp. This is an intergenerational event as many volunteers from church will be guiding the children too. All staff working with children will have passed background checks and received training.
The games and activities will use your mind, body, and spirit as we create, celebrate, and serve together. God counts on us to serve with love so there will be service projects each day.
We’ll be outside unless it is simply pouring. Please dress for the weather – with shoes meant for walking and hiking and digging around.
Everyday a delicious kid friendly nutritious meal will be served at noon with lunch and activities wrapping up by 1 p.m.

Children ages 4 to 6th graders are welcome to join in
REGISTRATION AND PARENTAL CONSENT FORM /MEDICAL RELEASE FORM
(must be completed for all participants & camp volunteers under the age of 18. (Use one form for all the children in your household!)
Consent for Participation, Medical Treatment, and Photo Release:
I am the parent or legal guardian of the below named children and I give permission for my child(ren) to attend Vacation Bible School at Riverview Estates hosted by United Presbyterian Church and participate in all VBS activities.
In the event of an emergency, I give permission to transport my child(ren) to a hospital for Medical treatment. I wish to be advised prior to any further treatment by a doctor or hospital. In the event of an emergency, if you are unable to reach me, I give permission for the below provided alternate emergency contact to give consent for treatment. I acknowledge that United Presbyterian Church nor Riverview Estates will not be responsible for medical expenses Incurred nor held liable for accidents and injuries that could occur during the event.
I give permission for the below named child(ren) to be photographed during VBS and for the images to be published but not limited to internet and church publications, without liability or limitation on my minor’s part.
